Insurance Information

For patients with insurance that we are in network with, as a courtesy, we will assist you in processing your insurance claims. In order for our office to file your insurance claim, you must bring proof of insurance at your first appointment and then once a year thereafter. You will be responsible for the entire cost of your first visit (you will be reimbursed after payment from your insurance carrier has been made) and thereafter for your uninsured portion (co-pay and/or yearly deductible) at the time treatment is rendered.

All incurred charges are ultimately the responsibility of the patient regardless of insurance coverage. We must emphasize that as your chiropractic and acupuncture provider, our relationship is with you, our patient, and not with your insurance company. Your insurance plan is a contract between you, your employer and/or the insurance company. Our office is not a party to that contract or any possible restrictions.

We are currently in network with and accept assignment for the following insurance companies and programs:

  • All Auto Insurance PIP Providers

  • Medicare

  • Blue Cross Blue Shield - All PPO's

  • Florida Health Care - Triple Options

  • Volusia Health Network

  • Cigna - All PPO's

  • Humana- All PPO's

  • Coventry

  • Veterans Administration - pre-certification required
